How can i set up a loan?

i want to loan a friend some money but am scared bec a verbal mutual agreement has not always worked in the past. i have three friends who never paid me back and i think it may have ruined a couple of my relationships. i want to help this other friend because i really trust him. is there a form (like a lease form) that i can have him sign (i know he has property and i know if you get a loan from a bank you have to allow them the potential right to put a lein on your property if you default....but i dont know how that works and i cant hire an attorney bec of time and schedule issues. besides i'd rather do it myself if its simple.)
